Born in Chicago, Illinois, on September 11, 1961, she is the sister of actor Michael Madsen. She gained significant early attention playing Princess Irulan in David Lynch's 1984 film Dune and later received critical praise and a Saturn Award for her starring role as Helen Lyle in the 1992 horror classic Candyman. Madsen achieved her highest critical recognition for her portrayal of Maya Randall in the 2004 film Sideways, which earned her nominations for an Academy Award and a Golden Globe Award for Best Supporting Actress.
